These forms should be submitted to the UCS Data Base Administration Group at 2318 Dunford Hall, Knoxville, TN 37996.
When your request has been processed, you will receive your IMS ID and initial password in the mail and you will be notified by someone within the Controller's Office.
Other Financial Information is available on the AS/400. Access to these systems may be requested by completing the AS/400 Authorization Request.
You must submit this form for access to any of the following Financial/HRIS systems:
Indicate on the form which of these systems you need and list the specific accounts, departments, etc. that you wish to access and have the person(s) responsible for those areas sign the form. You will be granted access to these accounts for all of the Financial/HRIS systems to which you have access. This form can be used to request access for a number of other University of Tennessee mainframe systems in addition to the Financial/HRIS systems.
